Per industry regulations, we are required to offer you the option to purchase travel insurance. Therefore, we require that all of our clients complete, sign, and submit this Travel Insurance Form. By signing this form, you indicate that you have been offered the option to purchase travel insurance, and that should you choose not to purchase travel insurance, you accept all responsibility for any expenses that are not included in the trip cost; injury; damage; delay; or loss that may result before, during, and after your trip on the account of any conditions, actions, or omissions that are beyond reasonable control.
Emergencies can happen when you least expect it. We highly recommend that you protect your vacation with a travel insurance policy. Cruise lines, airlines, hotels, and tour operators all charge high penalties for cancellation, ranging from the full deposit to no refund at all. Delays, baggage loss or damage, and unforeseen trip interruptions are all potential negative impacts to your vacation plans. Another important fact to bear in mind is that most health insurance policies will not cover you when you are out of the United States. Your Travel Insurance may protect you in the following areas:
Please also note that in order for you to benefit from pre-existing medical condition coverage, your policy must be purchased within 14 days of paying the first trip deposit. There is no added cost to the policy for pre-existing coverage.
It is also important to understand that you do NOT have coverage until you have an active policy. Pre-existing coverage will not apply to world events such as strikes, natural disasters, terrorism, etc.; nevertheless, such events are often covered in travel insurance plans.

I understand and have been advised of the benefits of Travel Insurance. I acknowledge that I do not have any travel insurance coverage until this form has been signed and submitted, and my purchase of Travel Insurance has been confirmed.
Should I decline this protection, I understand that I will assume all financial responsibility for any expenses incurred as a result of being unable to travel, or travel disrupted for any reason as well as any medical expenses. I also understand that I must purchase Travel Insurance immediately to obtain maximum coverage. In order to be covered for any pre-existing conditions, travel insurance must be purchased within 14 days of paying the first deposit, and it must cover the full cost of all non-refundable trip arrangements.
Note: Allianz will refund your premium if you cancel your insurance within 10 days of purchase, and have not filed a claim or departed on your trip.
